Private and Confidential Patient-Centered Therapy

Patients participate in all therapy decisions and communicate directly any questions, concerns, issues, and topics for future sessions and requests for further services or resources.

Patients receive considerate, respectful, confidential, and professional care at all times.

Referrals

Referrals to other sources of help and support are available such as support groups and other  professional practitioners in specialty areas.

Telehealth video and phone sessions are available. In-office visits scheduled when possible.

Client forms will be emailed before your first visit. Please take your time and fill out the forms as completely as possible. This information will be helpful in understanding your reason for therapy and to assist with recommendations and support.

During your first visit, I will go over your registration forms. Then an interview will follow, which will cover topics that are important to you (and your partner if you are in a relationship).

If you are in a relationship, the history of your relationship will be discussed from when you met and up the present time. To better understand the experiences, issues, patterns, and ongoing issues you are experiencing. 

At the end of the session, there will be a summary and recommendations for ongoing regularly scheduled appointments.  Questions, comments, and ongoing communication is encouraged.

Patient Forms

Please submit a copy of your driver’s license including photo ID and your medical insurance card. Front and back sides of the insurance card if you are from out of state and both sides of your ID if you have military insurance. 

Payment and Insurance Accepted 

Verify your health insurance coverage with your insurance company before you arrange your first visit. You are responsible for all charges. You’ll be asked to send a copy of your insurance card and driver’s license for billing.

Copays & Deductibles will be collected at the time of services.

Credit card, check, or cash is accepted for payment. 

Plans for Patients Who Pay Cash

Cash cost per single session: $175.

Package and premium plans for multiple sessions available.  Please request information if you would like to know more about these package plans before your first session.

Rescheduling and Cancelling Appointments

Patients must call by 10 am the day before their appointment to avoid a $50 fee.
